PPO plans are the most popular plan that doctors, clinics, hospitals, and pharmacies contract with. One of the reasons that the PPO plans are so popular is because they pay the doctors a discounted fee for service based on their fee schedule. PPO plans offer a low premium that has a higher deductible or the other option is a high premium with a lower deductible. The patients are responsible to pay a copayment, and there is also a yearly deductible that the patient has to pay out of pocket. If a patient sees a doctor outside of the network without a referral, the plan will pay less and the patient is responsible for the remainder of the fee. Patients have their choice of providers, but if the patient goes to a out-of-network provider it will cost more. One thing to remember though is that all non-emergency services require pre-authorization.…

Preferred provider organizations (PPOs) are a private plan which is the most popular, followed by health maintenance organizations (HMOs). PPOs sometimes pay participating providers based on a discount from their physician fee schedules which is called a discounted fee-for-service. With a PPO the patient pays annual premiums and often a deductible. The patient has two choices with the first being offered a low deductible with a higher premium and the second being a high deductible with a low premium and they always pay a copayment at the time medical services are rendered. Consumer-driven health plans (CDHPs) combine two components with the first being a high-deductible…

Managed care contributes a vast amount to the healthcare system. Without managed care systems I don’t think health facility, insurances agencies and companies would manage. Agree, professionals are to adapt and accommodate changes based on contracts between managed care systems and other agencies. In my opinion, the technology advancement within the health care system has improved tremendously in a positive way. For example; when I switched my daughter primary doctor around shot time, me being old fashion handed her the paper shot record. She replied “ma’am it already imported into the system; we don’t use the paper anymore.” This is totally a successful and less stressful outcome on a person if it was to get lost or misplaced. These…

Believe that each plan offers something for each provider and consumer. The plan that is best for one person may not be for the other person. There are different plans to help people that have different preferences. As the consumer you need to find the plan that is best for you and your finances. As the provider, the plans make money for the insurance companies on different levels, so with this in mine there is money being made on each plan by the consumer. Individuals that depend on going to the doctor a lot may want to consider having a plan that pays more and covers more. For the past several years, the health care and insurance industries in America have been undergoing significant reform in order to rein in the high cost of delivering health care services. Managed care has become a cornerstone of this process (Strickland, 1995). The case management industry (with its focus on cost containment, managed competition, and quality care) is playing an increasingly important role in the managed care environment (Owens, 1996). According to Mullahy (1995a), the number of case managers has risen astronomically in recent years. These individuals come from diverse professional backgrounds and practice settings that include nursing, rehabilitation counseling, and social work.…

Third-party payers include private insurers, such as Blue Cross and Blue Shield, and public (government) insurers, such as Medicare and Medicaid. Third-party payers use several reimbursement methods to pay providers, depending on the specific payer and the type of service rendered (Gapenski 2013). There are two classifications for payment-fee-for-service and capitation. Fee-for-service means exactly what it says. So, the more services done, the more the reimbursement will be. Reimbursement is based on charge, cost and prospective payment. With charged-based reimbursement, the doctor is in control. The payer pays based on a rate schedule, which the doctor creates. Cost based reimbursement guarantees the doctor will be paid if the service is delivered. Prospective payment reimbursements are determined before the service is done. The charge can be per procedure, diagnosis, per day, bundled payment, etc. Capitation is a bit different. The doctor is paid a fixed amount (monthly), regardless of the services completed. Coding on reimbursement does not only affect the doctor’s pay. An inaccurate code could result in the doctor being underpaid by thousands of dollars. It also affects the patient’s quality of care. It is very important that doctor document every detail regarding the…
