The development of these prevention strategies allows for a successful reduction of CAUTI nationwide when implemented correctly in the medical filed.
Prevention strategies such as checklists, verbal or written reminders, sticker reminders on the patient’s chart or catheter drainage bag, or an electronic reminder are a few suggested implementations. Another consideration for implementation is a physician assigned stop order, prompting for the removal of the catheter after a certain time period.
Since the use of urinary catheter is often unnecessary, the CDC has provided guidelines for appropriate indications including: acute urinary retention or bladder outlet instruction, a measurement for urinary output in critically ill patients, perioperative use for selected procedures, open sacral or perineal wounds in incontinent patients, patients requiring prolonged immobilization, and improved comfort for end-of-life care. All the evidence-based prevention strategies discussed has offer significant opportunities for improving patient safety and achieving prevent goals to limit the risk of CAUTI (Knudson,
2014).
Patient Related Education Misses was seen by myself on November 14, 2015 to perform a urine specimen from her Foley catheter. Misses’ catheter was placed due to a broken hip, which has left her immobilized and bed-ridden. As per the above guidelines by the CDC, immobilization is an appropriate indication for an indwelling urinary catheter to be used. At this time I would not suggest the care plan to be changed; however, I would recommend close monitoring and urine culture as I preformed today. Misses urinary specimen appeared cloudy, hematuria and strong odor, which are all symptoms that can indicate misses could possible have a urinary tract infection. Once the culture comes back and a CAUTI is indicated, misses will need to be prescribed an antibiotic and urinary catheter will need to be changed. Once misses urinary tract infection is eliminated, I would also recommend that proper prevention strategies be implemented once again and signed off on to make sure misses stays CAUTI free.
