Dr. J.K McClean of cardiology is evaluating his heart condition. The patient has had the recent onset of hemoptysis. He was treated for tuberculosis in Bangladesh 15 years ago. This has prompted the concern of weather his treatment of tuberculosis was adequate or if weather there is another cause of his hemoptysis. The duration of his tuberculosis treatment was apparently adequate, according to his wife. But no records are available. In addition, the patient had thrombosis of the axially artery treated last year at Hillcrest. He had an embolectomy and has been on Coumadin since. INR is significantly elevated at 16. None the less, because of the cavitary lesions that are seen in the right and left upper lobes, the possibility of tuberculosis has been raised. Ancillary history was been given by the wife, Nupaul, with the patient translating for her from the Indie language.…

The high turnover of licensed staff in nursing homes, it not only effects the resident’s quality of care and the morale among staff members, and is also very costly to the facility. The purpose of the study was to understand what influences nursing staff to stay at a LTC facility. There have been studies conducted in the past but they were primarily done with nonlicensed staff or within the acute care setting. The goal of this study was to gain insight on how to retain nurses in LTC facilities. This study is significant as administrators in LTC facilities have a hard time attracting and retaining licensed nursing staff. When there is a high turnover of staff, it can affect the care of residents and morale of staff in a negative way, and is very costly.…
Summary: This article informs the reader of the shortage of nurses. The article mentions, as in the past, nurses are still putting in long hours, caring for more patients, desperately trying to stay on top of things. There is a new nursing shortage, different from the old one. The reasons of the shortage are examined. Fewer people are entering the nursing field, a general dissatisfaction of many issues, and a high burnout rate. The article consists of three main reasons for the shortage, each of which go into detail about a certain issue. The new shortage requires unique solutions that will fix the underlying issues, not just solutions that will help in the short run. The solutions of which that were mentioned was needing stronger, professional work environment, adequate staffing, and salaries and benefits to that equal of their responsibilities. To help combat the shortage, federal and state governments are presenting legislation to improve the working environment and helping to pay nursing tuition. The nursing research question that is being asked in the large picture is “Do Magnet principles affect nursing turnover and satisfaction?” In looking at this question by the method of a systematic review, the following article was found, “Factors influencing job satisfaction of front line nurse managers: a systematic review” (Cummings& Lee, 2008). This review is pertinent to nursing research in the fact that it addresses a different set of nurses whom support those who are the direct caregivers. If they are not satisfied in their position, it could affect those whom they lead. As healthcare heads into its next phase, those who lead them are a barometer of the staff. The newest governmental standards address patient safety, quality, and satisfaction, in a financial context for healthcare institutions. While this is a patient barometer, there are many studies that indicate that staff engagement and satisfaction, and even lower turnover affects patient outcomes. So if one follows that line of evidence, to further research how the managerial satisfaction could affect both factors. The article discusses the vital role that front line managers play as a liaison between the staff level and administration. The study itself also notes the limited amount of research on this subject, as most research is more focused on the front line nursing staff satisfaction.
