The patient continue to discuss about her prior DCF case as she still parent through guilt and also being in a hospital for an abscess was an awaken for her for
the moment. This writer strongly advised the patient that she needs to show commitment to her recovery because the sole purpose of her seeking help at a methadone clinic to get the help she needs to help her with her addiction, but if the patient continues to address the problem head on about the usage of the cocaine, there will be more consequences she will face. The patient informed this writer about her mental health therapist addressing the issues and it appears that her therapist is saying the same thing as this writer and the patient understands the concern, but not following through with commitment. This writer gave the patient an ultimatum that if she get another positive urine screen, she will be placed on a treatment violation and suggested the patient about attending an IOP and NA meeting. The patient is receptive to attend those meetings but her work schedule is a problem; however, the patient is willing to explore NA meetings.
A-Based on this writer's assessment, the patient is still at the contemplation stage of change as the patient is aware of her drug use as a problem, but needs a strong commitment for her recovery process. In addition, the patient needs to give herself credit for fighting for reunification for her daughter during her DCF case. The patient appears to be alert, oriented, and well-mannered. There was no evidence of SI/HI.
P-The patient will continue to adhere to the program policy. The next scheduled session is on 03/31/2016 at 9:30am.
