I felt good to be able to provide continuous, comprehensive care through disease management, health promotion, health education, and preventative health services in the clinical. As a clinician, I have been treating the family as a whole for better outcomes and involving them in any necessary decision-making process. I have also learned that verbal and nonverbal communication is crucial.
Furthermore, a patient’s evaluation of a clinician’s trustworthiness influences the patient’s willingness to disclose psychosocial or other barriers to treatment effectiveness, and his or her expectations of treatment efficacy and motivation to adhere to the treatment. I have grown professionally through the clinical and my ability to critically think is evolving every day. In addition, with experience and confidence, I have learned to critically think as I collect, analyze, and synthesize, evidence based patient information to provide positive patient outcomes.
2. Describe at least two things you felt did not go well during this term’s clinical experience.
As I started clinical, I was anxious and I did not know what to expect. The feeling of not knowing, what to expect initially and being my first nurse Practitioner clinical was a little …show more content…
overwhelming. However, I have become better as I critically think and interact with the patients.
As a nurse, transitioning to a provider took a moment, but I realized that my nursing knowledge in conjunction with the knowledge I have acquired as a nurse practitioner student and the ability to critically think while applying evidence-based practice in the practice is making the experience great.
3. What new learning came about because of both the good and not so good experiences? What new skill or insight was developed?
The patient presented with a rash and after assessment I was not sure of the diagnosis. I was not pleased and felt not so good. I have seen several patients with shingles and I am able to diagnose knowing what to look for when assessing. No two days are similar and I am learning more every day. I also see myself researching further if I am not sure how to exactly diagnose a problem. Learning is constant and ongoing and I am enjoying the challenge of continued learning and application of knowledge and evidence based for positive outcomes. Being in a position to manage patients with chronic conditions and provide education has been rewarding.
4. Thinking back on the term, reflecting on your actions or decisions, what would you change or do differently?
Thinking back on the term, one thing I would change is to understand that as a student nurse practitioner, I will not be proficient or know everything and that I will always refer to the evidence based resources to provide quality care. There will be a need collaborate with other providers for positive patient outcomes
5. Evaluate at least one decision or intervention or plan that you initiated.
The patient presented with UTI and I was able to communicate to the preceptor my assessment, diagnostic testing, diagnoses, and treatment.
I provided patient and family teaching on maintaining good hydration, good hygiene, wiping front to back, wearing cotton underwear, avoiding wearing tight-fitting pants, potentially irritating feminine products such as powders and perfumes, no douching, taking shower instead of tub baths in addition to performing kegel exercises. I also conveyed the importance of completing the course of antibiotics even if they feel better and to follow-up if no improvement or if symptoms worsen. My plan was
evidence-based.
6. Identify at least one Learning Goal for the next clinical course and what resources will you use to assist you in meeting this goal.
My learning goal is to manage and develop patient plan independently with my preceptor validation. I want to continue improving on obtaining patient history, establishing differential diagnoses, examination skills, and patient teaching and anticipatory guidance. I will use Dynamed plus and Lexicomp the evidence based resources in addition with my physical assessment textbook to meet my goal. I will also ask my preceptor questions and clarification and seek her guidance throughout the clinical course.
