E. F. Medical Case
E.F. is a 45-year-old male African American patient with end stage renal stage. He is divorced, no children, family members reside outside of the state. E.F is on hemodialysis treatment for five years. His diagnosis includes, cirrhosis of the liver, Hepatitis C and HIV.
E.F. comes to emergency room almost twice a week because of shortness of breath, swelling, hyperkalemia and at times positive ETOH. E.F. always fail to show up in his scheduled hemodialysis in outpatient center. He mentioned the distance of the center and transportation issues. To him its more convenient to call paramedics and have his dialysis in emergency room. According to his record his center was changed multiple times by social worker so it will be easy access for him.
