Electronic health records (EHR) are being applied in healthcare systems for the dual purpose of improving healthcare quality and decreasing operating costs. More than 50% of doctors and other eligible providers have received Medicare or Medicaid incentive payments for implementation of EHRs and over 80% of hospitals successfully incorporated the EHR by the end of 2013 thus getting incentive payments (Business Wire, 2013). Emergency departments are faced with problems such as overcrowding, job stress, skeleton crews and work flow interruptions. The main concern for introducing the EHR is if it will improve…

Alexian Brothers Medical Center is located in Elk Grove Village, IL, a northwest suburb of Chicago, IL. They are a 387-bed acute care hospital, with more than 900 physicians on staff, representing more than 80 medical and surgical specialties. They treat more than 18,000 patients annually. Instead of purchasing an existing EHR system, they opted to create their own, AlexiCare, to assist with the handling and processes involving medical records and patient care.…
Computer technology continues to make rapid advances in healthcare facilities. Many healthcare facilities have used computer programs for administrative functions such as payroll and billing. Electronic health record (EHR) systems have the potential to transform the health care system from a mostly paper-based industry to one that utilizes clinical and other pieces of information to assist providers in delivering higher quality of care to their patients.…
Health care tools has changed to organize better care for patients. Doctor’s use computer’s or laptop in the office and exam rooms to enter electronic health records (EHR). The EHR makes it easier for the patients to receive better organized care along with better organized health statistics.…
What I can see now in the United States, is a race between, EHR, EMR, and PHR. Electronic Medical Records or EMRs are the electronic versions of classic paper charts that are still used by some clinicians who are still not 100% compliant and use for diagnosis purposes. While Electronic Health Records or EHRs have a wider scoop of a mission, for primary doctors can follow their patient’s journey of care through internet connections, but also allowing other clinicians to have access to that information for the same purpose of care. And Personal Health Records or PHR that allows patients to keep their own medical records online and enable them to control everywhere without visiting a clinic. Wherever patients travel and need medical care, they can retrieve their own records using the Internet. Whatever their purpose, now that computer system is widely used in medical practices, than in paper-based system, everything that used to be handwritten by healthcare providers and staff, including medical biller and coder, is now entered into a computer, directly into EHRs. And with this system, EHRs can increase the efficiency of staff members in the practice and at the same time improve the quality of care for the patients. No more time spent looking for charts or missing information. Multiple staff members with appropriate access privileges can view and modify a single patient’s chart simultaneously. No one has to wait for a chart to mail or deliver…
As a group, we are encouraging the physicians to use the technology provided for the benefit of our patients and for this organization. We will identify that electronic medical records (EMRs) and electronic health records (EHRs) is a valuable tool, provide the rationale for why EMRs and EHRs are important, and the legal and ethical aspects. We also will talk about some solutions to put in place to help physicians comply with this technology.…
The staff employed in a medical facility depends on many things to keep the quality of patient care in the positive and efficient. Physicians and nursing need the current and most recent information on a patient to ensure this. The EHR system makes this more possible because the rate for exchange of information increases with this system. It not only provides more accessibility to a patient records from the other facilities and departments that also carry their medical information, but can also ensure the patient will get the best care possible by keeping the staff up to date with their medical information using these coordinating methods. Some of these methods include information such as diagnostic reports where they can be uploaded into the system instantly once complete and offer a faster rate of review for the staff. Did I mention that this system also notifies the assisting staff when these reports are ready for review? Imagine how many live could be saved or changed with this? (Dickerson, Sensmeier, 2010).…
The technological trends that face health care management will change how physicians and patients communicate with one another. Having the knowledge in the field of Health Information Technology (Health IT) that includes Electronic Health Records (EHR) will increase communication between providers, physicians and patients. This type of technology is not only efficient but offers the patient access to care and information than that of past traditional methods in the delivery of care. It is important that there is an understanding of Health IT and other technologies as it will affect the traditional methods of collecting, monitoring as well as providing new ways of delivering health care…
Electronic Health Records was developed around the 1960’s and 70’s. An Electronic Health Record is a digital collection of patient health information compiled at one or more meetings in any care delivery settings. A patient’s health record includes their vital signs, past medical history, demographics, their laboratory data, immunizations, progress notes, problems and medication. EHR is often referred to the software platform that manages patient records maintained by a medical practice or hospital.…
EHR is a new and improve digital version of a patient’s physical paper chart. EHRs is a patient-centered system that makes information available via the computer system, and most of all, it is secure, safe and can only be used by authorized users. Even though EHR involves medical treatment and patient medical history, the system is created to go beyond normal or basic clinical data collection in hospitals, private doctors and other healthcare facilities across the nation. As a Consultant, I will have to come up with a strategy planning process, which will make everything fall into place. When it comes to setting up a hospital or updating old technologies within a hospital setting, the administration work in conjunction with the staff such as the doctors and nurses and also alongside the CEO Mr. Johnson, who is the new executive who brought in recently in the Tewsbury hospital.…
Health Information Exchange and Electronic Health Records are essential for the continuity of care for patients and necessary for healthcare providers to be able to share documentation and communicate information readily with Electronic Health Records (EHR). This demand has led to the need to promote healthcare standards and to allow for use of electronic health records and health information exchange to promote quality health care. The purpose of this paper is to provide an overview on the role of Health Information Exchange and Electronic Health Records in provision of quality healthcare.…
Electronic Health Records is the technology that I find most beneficial at work. It helps nurses, doctors and other healthcare providers to access patient information literally for 24 hours a day, seven days a week. EHR allows for a better coordinated care for all patients. The information can be transmitted immediately to other providers. EHR allows nurses and doctors to navigate through patients’ data that is far better than pulling charts on the cabinets and searching through pages of the chart to obtain one or two information. The beauty of EHR is that multiple healthcare providers can access the same records at the same time. There are some short comings about electronic health records such as when the system is off line, documentation…
The Health Insurance Portability and Accountability Act, originally passed in 1996, gives the US Department of Health and Human Services the authority to enforce regulations involving the use of medical records or other health information. According to the department's website, consumers filed almost 50,000 complaints regarding health care privacy between 2003 and 2009. Patient Rights Under the federal HIPAA Privacy Rule, patients have several rights regarding their medical information. The privacy regulations apply to many kinds of health information including patient medical records, electronic health records, billing information and conversations between doctors and other health care providers, according to the US Department of Health and Human Services. Patients' health care providers must provide them with notification explaining how their medical records and other health information will be utilized. According to the Department of Health and Human Services, medical information cannot be released to employers, an advertising or sales company or any family member or friend that has not been designated as a personal representative for the patient. If patients consent to having their information given to another party, they must sign an authorization form that clearly explains who the information is being released to and for what reason. HIPAA regulations also give patients the right to receive one annual report from their health care provider that documents who their information was shared with. For example, health information can be released to government agencies for public health purposes without a patient's direct permission. Patients, and in some cases the court system, can appoint a patient representative to receive health care information on their behalf. Parents and legal guardians automatically have this right when dealing with minor patients. In addition, individuals that have power of attorney or are executives of a deceased patient's estate can be…
