3rd & 4th YEAR UNDERGRADUATE AND MASTERS DEGREE PROJECT RISK ASSESSMENT
1. A project risk assessment must be completed by the student and approved by their Supervisor before any work can commence. Once completed, the risk assessment form must be handed in to the Departmental Officer, 7th floor Departmental Office for record keeping. It will then be examined by the Departmental Safety Officer (DSO) Kenneth Tong.
Note. Type in the information, handwritten forms will be returned if they cannot be read.
2. Copies of the project risk assessment should be kept by the student (in their Laboratory Log book), project the supervisor, Departmental Safety Officer and Teaching Lab / Clean Room technicians should your project be carried out there.
3. The document must be completed for all projects, whether the project is desk based, laboratory based, clean room based or off site.
4. If lasers are to be used then the departmental laser safety officer, Dr. T. Kenyon must be consulted.
5. Risk assessments must be reviewed, and where necessary revised, at least once every 12 months, and whenever there is a significant change - even if this is a continuation of the project.

Section 1. How to complete the risk assessment form
The form is to be completed (legibly) as follows:
i) The student is to complete all the boxes in section 2, including: title of project, description of project, Supervisor details and DLSO approval if required. ii) The student must complete hazard identification with help from the Supervisor.
