A SOAP note is a written documentation the physician takes when a patient comes in for a visit. In a SOAP notes the physician seeing the patient write down how the patient is feeling and the symptoms they are experiencing. Then they examine the patient and take notes on what they find. Last they say what that they believe is wrong with he patient and then provide a plan after their visit. Whether they need to see another doctor or schedule a surgery. An example of improper medical documentation would be if a physician were to write down the wrong abbreviation for a test the patient needs. Or when they are examining the patient they miss diagnosed the patent and they receive the wrong treatment. If either of these actions were to happy the patient may be given medication that can make they issue worse or they could also have to pay for test they don’t actually need.
