The most recent initiative is to complete and identify at-risk patient during admission. A history of a previous fall automatically sends a red flag when a patient is admitted to the facility and during inpatient stay. Patients with mobility problems are high risk for injury. Some of the identified intrinsic features associated with anticipated physiological falls includes polypharmacy, lingering effects from primary illness, and confusion. According to the information obtained from the fall champion on my unit, successful fall prevention strategies such as conducting initial assessment within two hours of patents admission help identify those patients at highest risk for a fall helped implement fall prevention intervention faster than by not using it. Sustaining serious injuries from a patient fall kept the person in the hospital longer and required further treatments that could have been avoided in the first place. Extrinsic factors for fall precaution that were not being utilized included non-use of bed alarms, lack of use of non-skid socks and placing the patient in a room far away from the nurse’s …show more content…
Initiating an ongoing awareness campaign that increases knowledge among staff, patients and families of the need to prevent falls and simple steps that can aid in fall prevention. A goal is to create a culture of openness where any department can speak about their suggestions for improvement and together, strategies to reduce patient falls can be actualized.
Bed safety: There are contemporary beds with bed sensors, which are networked to the nursing station. This will alert the nurses and other care team members when a patient attempt to get out of bed, and as a result, staff will be present, and fall is avoided.
Environmental safety: The rate of falls can be reduced by ensuring that care environment is clean and free of hazards and objects that may lead to falling. It also includes ensuring that staffs are positioned in the milieu to make sure that they are available to monitor the environment and to have quick access to patients that may need help with transfers and ambulation.
Post fall Huddle: Conducting post-fall management analysis that incorporates a system of honest, transparent reporting, involves staff from all levels and, if possible, the patient, and reviews if appropriate interventions are in place, why the fall may have occurred and strategies for preventing future falls.
