A commissioned service means that the care, support or supervision has been arranged and paid for on the service user’s behalf. This will be paid for by the local authority health and social care department. Commissioning is a cycle when a local authority’s, at a strategic level, decides which care and support it should design, deliver, monitor and evaluate through a contractual arrangement. It is a way of assessing needs, planning and prioritising, purchasing and monitoring health services, wanting to get the best health outcomes. A lot of older people want to remain at home, and again, there are not enough affordable home-based care options. Communication in adult care services can be challenging, the coordination and integration of care services is not always effective meaning the transitions from care providers is not always smooth meaning gaps can be found in care. There is a growing need for specialised dementia and mental health care which is not about housing individuals but ensuring care services are tailored to the unique needs of individuals with a cognitive impairment. More dementia programs and dementia care training are required that are affordable to address this gap.
