Use the matrix below to identify and describe 5 different options available for long-term care populations across the continuum of care. Each description must be at least 50 words. Also indicate the level of care for this option along with a brief explanation of how this level is different from the others.

Description

Example: This is a facility that allows clients to live in a residential setting while having personal care services and assistance, usually at all hours. Clients will generally need assistance with such functions as eating, bathing, dressing, toileting, ambulation, or help with medications. Clients living here are unable to function without this assistance.
Out patient care is care that is provided to individuals that can come and go (no over night stays). Out patient care is for people that need assistance with things such as physical therapy, speech therapy, as well as occupational therapy. Other types of outpatient care are chemotherapy centers, pain management centers and dialysis.
Home health care is care that is provided to individuals in their own home. Patients receiving assistance from skilled nurses, physical therapist, speech therapist, or even occupational therapist. Clients also can receive assistance with ADL’s such as bathing, dressing or even medication monitoring. Care is provided to anyone who is qualified.
This is a type of facility is for elderly adults who are still able to take care of themselves. This type of facility offers opportunities for socialization, support and actives. There are some cases were individuals are still able to receive homemaking services such as running errands, laundry, and housekeeping.
Nursing homes are facilities that provide skilled care in a residential setting

Outpatient care may be provided in a doctor's office, clinic, the patient's home or hospital outpatient department. Outpatient treatment in a doctor’s office or clinic, often supplemented by medications administered at home, remains the norm for most routine care. Thanks to advances in treatments and technology, many tests and surgical procedures formerly conducted in the hospital can be done in an office setting. Outpatient care also provides the norm for most mental health and chemical dependency treatment. Inpatient care is care given to a patient admitted to a hospital, extended care facility, nursing home or other facility. Long term care is the range of services typically provided at skilled nursing, intermediate-care, personal care or eldercare facilities. Inpatient hospitalization makes sense for major diagnostic, surgical or therapeutic services, where the patient’s condition or response to medication must be closely monitored. In the case of mental health treatment, a hospital stay may make sense if the person is suicidal or self-destructive or poses a threat to others. In the case of chemical dependency or alcohol treatment, a hospital stay may be needed during the detoxification stage to monitor symptoms during withdrawal.
