Hospital Observation Status
• Observation status is when you need short term treatment care in a hospital. It allows time for your doctor to find out what is causing your symptoms.

• Medicare allows you to stay in the observation status from 24-48 hours. After this time, you will be admitted or discharged.

• Medicare clearly states that an inpatient admission is allowed only if the care you are receiving is the type of care that requires you to be in a hospital.

• Medicare does not allow doctors to order an inpatient stay because a patient or family members requests it or just because you need rehabilitation in a skilled nursing facility (SNF).

• Hospitals and doctors MUST follow the Medicare rules.

• If you need short term rehabilitation in a skilled

In the article Medicare Under Observation, it is mentioned how unfair and ineffective the Affordable Care Act is for senior citizens in the United States. The article emphasizes on a letter written by a Medicare beneficiary who recently had gone to the emergency room to get treatment for a urinary tract infection and ended up having to stay for hours only to be told he was not going to be able to go home and he was being admitted into the hospital for observation which he claims that under the Affordable Care Act law Medicare will not pay anything on. This left him very puzzled and wondering how come he needed to stay over a period of two to three days. This being the case, he would have to be responsible for paying this medical bill in full…

In most cases, Medicare Part A will cover inpatient hospital stays, rehabilitation centers, and long-term care facility stays. For Mrs. Zwick, Medicare Part A would apply to the rehab and inpatient hospitalizations. Medicare Part A will cover Mrs. Zwick’s five day hospital stay and the first 20 days of her stay at the rehabilitation facility. Each day following the first 20 days of rehab, a fee of $144.50 per day will accumulate at the rehabilitation facility for a total of $2890.00 which the facilities will be responsible for due to the patient acquiring a preventable infection during her stay (Medicare.gov, 2012). Medicare Part B is similar to medical insurance; covering medically necessary services such as MD visits and services, outpatient care, durable medical equipment, home health services, and sometimes preventative care services. In Mrs. Zwick’s case, Medicare Part B would cover the prescribed walker or other assistive devices if needed.…

Regarding her moving to the skilled nursing facility, the cost will be covered by Medicare one hundred percent the first twenty days. After a hospital stay of at least three days, your mother's stay in a Medicare-certified Skilled Nursing Center can be covered at 100% for the first 20 days. For the next 80 days, if your mother had to stay that long Medicare Part A covers everything except the daily coinsurance. I'm sorry no one informed either of you that your mother got an infection while at the nursing facility. That information should have been disclosed to the both of you. Unfortunately because your…

Medicare Part A covers inpatient hospitalizations, skilled nursing facilities, hospice and home health care. The hospital, skilled nursing facility and home health care has to be a Medicare-certified facility and service. Based on Medicare Part A, Mrs. Zwick’s inpatient hospital stay of 5 days would be fully covered, according to Providence Life Services (2012) after the first $1,068. We all know that it is protected right that only the patient has the right to make their own healthcare decisions. The hospital is governed by federal law with Health Insurance Portability and Accountability Act of 1996 (HIPAA). “The major goal of this law is to ensure the protection of an individual patient’s health information while balancing the need to provide quality healthcare. The violation of this statute could result in substantial fines for the facility.” (Vergari, Los Angeles Lawyer)…

    Medicare Observation

Thank you both for your comments. As you have both mentioned the Medicare rule for observation is very complex and confusing. The intention of the rule is to set clinical criteria for observation, and rapidly discharge the patient in twenty-four hours. However, this does not always happen. Some observation patients can stay up to ninety-six hours before being discharged or switched to observation status. One of the problems is individual health care organization interprets the Medicare rule differently. Then if the patient had a managed care Medicare carrier, the rules may be different. This makes observation status for an organization nightmare to manage (Hockenberry, Parlato, & Ross, 2014). What I find even worse, observation increases the…

This reflection is based on my observations of the need for of effective communication skills and knowledge of nurses regarding the management of aggression in elderly patients with dementia. Clinician need to be equipped with seclusion, medication and de-escalation skills to be able to manage patient aggressive behaviours safely. I have chosen these particular observations after realising how fundamental they are in implementation aggressive behaviour management strategies in patients. While working in mental health I have observed how effective it is to communicate with a patient as a way of de-escalation during verbal and physical aggression incidents. Ford, Byrt, Dooher, & ebrary (2010) point out that it is imperative for health professionals…

I thoroughly enjoyed the opportunity to observe and participate in Missouri State University’s Speech Language and Hearing Clinic because I received hands-on experience and had the ability to interact with a client. Before entering the clinic, I was extremely nervous, but I was excited to begin working with a graduate clinician and client. During my first experience in the clinic, my graduate clinician, Bob, taught me how to collect data. Bob was an excellent mentor because she acted professionally and was open and willing to answer questions. She took the time out of her busy schedule to visually show me her session plans and SOAP notes, so I had a better idea of the paperwork format. Bob had a supportive and positive attitude toward…
