Letter of Consent and Undertaking

In accepting the offer of Huajing 神州搵好工2013, internship period from __________ (dd/mm/yyyy) to __________ (dd/mm/yyyy) jointly organized by the Huajing Society and The University of Hong Kong, I, _______________________(full name in English), hereby, declare that I have explained with details to, and sought the consent with, my parents and family about my participation in the captioned internship and we are fully aware of the possible benefit and risk pertaining to it. We will not hold the organizers, host organizations or any of their staff members liable for any loss, claim, proceeding, damage, injury or illness for which they are entitled to be indemnified under any policy of insurance.

I read, understand, and agree to abide by, my obligations under the Terms and Conditions stated below.

Terms and Conditions

1. I, the PARTICIPANT, shall immediately report all potential issues, as well as abnormal or unsafe conditions to the organisers by phone or e-mail. PARTICIPANT is responsible to notify the organisers of all changes to the position description or PARTICIPANT's information on record within 2 business days. If for any reason HOST and/or PARTICIPANT discontinue participation in PROGRAM before or during the internship, PARTICIPANT shall immediately notify the organisers within 2 business days.
2. Good Health: PARTICIPANT must be in good health prior to the internship commence. PARTICIPANT is responsible to report and keep the organisers informed on all preexisting and new medical conditions at all times.
3. Denial of Visa Application. PARTICIPANT understands and agrees that the government agencies having jurisdiction over immigration matters exercise discretion in administering their responsibilities, and therefore the organisers makes no claim or representation with respect to the likelihood of success in any visa application or the time in which a particular application is processed.
