When a patient is referred to home health, the nurse performs a face to face encounter to evaluate the patient in the patient’s home environment. The nurse obtains the patient’s health information which is collected to establish goals for the patient’s best outcome. The patient and family are involved in the decision making of the set goals to accomplish. The patient and the nurse develop a plan to follow, including interventions to educate the patient to attain set goals. The nurse may need to refer the patient to other disciplines such as physical therapy, occupation therapy, speech therapy, aide, dietician, pharmacist, or social worker to accomplish identified goals. The plan of care is collaborated with the physician who is following the patient for the home care episode. The plan of care is adjusted as the need arises for the patient to meet the goals established. I have assisted many patients and families in obtaining their goals over the years to have a healthier life style when I was working in the field as a staff nurse. I never really noticed of using any theory in home care, but now once I am learning about it, I can relate with the theory of King’s goal attainment theory. I believe one can accomplish anything if they set their minds to it. I encouraged patient’s to be a part of the plan of care and in the decision making of their health. I involved the family, if the patient was agreeable to it or if the patient was not able to make decisions on their own. If a patient was not able to meet a goal, I then changed the plan of care with communicating with the patient, family, doctor and other disciplines to accomplish the goal in a different way or set small steps to achieve the goal. I believe the King’s goal attainment theory fits the home care setting best as it follows a framework for the nurse to
