EXPLAIN PERSON CENTRED PRACTICE:
Person centred practice are ways of practising and organising tailored care, to provide what the service user wants, enabling them to live independently within their own homes and engage within the community.
It’s a way of ensuring that people with disabilities or the older adult have the same rights and choices as all other individuals.
We put the individual at the centre of everything we do, it’s their decision and theirs alone.
When I visit any new service user and do a care plan / support plan I act on what the person wants and how I can deliver it safely, occasionally compromises will be made to ensure the persons safety, dignity, emotional and spiritual well-being are met. I have to consider that this person might not have had care before and it may be over whelming. So I reassure them and try to make them confident and happy with myself and the service I provide.
I provide all care staff with the appropriate mandatory training to fulfil their rolls, to be compassionate and understanding.
CRITICALLY REVIEW APPROACHES TO PERSON – CENTRED PRACTICE:
Person centred planning refers to planning of service users to enable them to plan their own future, to be able to have the right service they need and to stay within their own homes as independently as possible. It gives each service user the right to choose their own care, care provider and level of support they need.
To make their own funding arrangement to take control of their own lives.
All care planning does involve families and next of kin. But the end decision is made by the individual.
The company aim is to make the service user comfortable with all staff that attend to his or her needs, to help them access the community, social support, network with others, make use of all other local resources.
I send out satisfaction surveys to ensure that the service users and families are happy with what we provide.
