Lidocaine Case Summary
DOI: 6/30/2016. Patient is a 48-year-old male janitor who sustained injury to his right shoulder, right hand, back (cervical and upper back) and right knee while throwing trash up into a bin. Per OMNI entry, he was initially diagnosed with strain to his cervical, back, right shoulder and right knee.
Based on the medical report dated6, IW was advised to start Lidocaine 5% patch.
Based on the medical report dated 01/18/17, the patient has initially received treatment, had x-rays and was prescribed medications. She has endured 6 PT sessions and was referred for an MRI to the right knee and shoulder last year. She is currently awaiting authorization for an MRI to her neck.
