Edward Alvarez Interview
The interview was conducted with HUB Representative, Edward Torres and with World Services Spanish Translator Mr. Palmes. He performed the translation from English to Spanish over the phone. The meeting with Mr. Alvarez occurred on 11-3-16, at 7:30 a.m., at the Kluft Holding Company, LLC.

Mr. Gallardo states he has over 20 years of experience as a tape measure machine operator. He works with a sewing machine that sews a tape around the two panels on the mattress to secure the integrity and quality of the mattress.

He said the process starts by dragging a 200-pound mattress over to the tape measuring table. He will then to sew the two panels together as he uses his fingers to squeeze the pieces of the mattress together while he uses the sewing

Gallardo confirmed that he is a full-time employee and works approximately 40 hours a week with added over time equaling up to 50 hours a week. He claims to work Monday through Friday, with the weekends off. He claims to earn $18.30 per hour and is supervised by Mr. Louis Montes.

He claims to know the proper procedures to report a job-related injury within 24 hours of the incident. He has attended some in-house safety training classes each month. He recalled from the classes which he took that he must immediately report any work-related injury or illness immediately to their Supervisor.

Mr. Gallardo maintains sometime in the middle of the year in 2012 he found that each time he was at his workstation working on mattresses, he started developing soreness to the top portion of his right shoulder. He said as soon as he began feeling the soreness, he self-mediated with icy-hot and other creams to subside the soreness which eventually developed into shooting pain throughout his right shoulder when he reported working the next day. He said the pain and discomfort he felt was from each time he would either push or pull on a mattress on to the sewing machine table and while he would have to flip the mattress sometimes on its side, with the use of his right hand which he considered his strong hand.
