The reporting party (RP) stated she received a message from Noni Levi (858) 382-7870 the sister of resident Molly Bradley DOB: 4/27/64. According to the message this past weekend (exact date unknown) while the resident was visiting her mother (name not provided) her sister Noni observed bruising on both top inner thighs. When Noni contacted the facility she was informed the facility was not aware of the bruises and if the resident has bruises it resulted from her day program. The RP stated the resident has lived in the facility for many, many years and has attended the same day program just as long. The RP stated the resident uses diapers and the toilet. Reporting party stated that on 10/17/17, a door was closed on her daughter, Aaliyah Chavez (DOB: 02/10/16) , her right index finger. Staff told RP that the child was behind another parent who was picking up their child and staff did not see the child go behind the parent. RP stated that she sought medical attention at Kaiser Urgent Care where it was determined that the child did not have a broken finger. RP stated that the child's nail is coming off. It was also reported that on 11/17/17 when she arrived to pick up her child, she accidentally bumped into another child (male unknown) and teacher Genesis grabbed the child by his arm and roughly pulled him away. RA would not talk with Treva. RA constantly asked Treva to leave. RA’s daughter & family kept talking to the RA about getting services in the home, especially wound care. Family provided information that the RA will have a physician to come to the home to treat the RA’s bedsores, a nurse to monitor the RA’s meds, and an aide for the RA’s personal care. In report #17-067-00223 the RA left AMA from Hidden Lake Nursing Home. The concern was that his blood sugar was extremely elevated, and he left without taking his oxygen. Treva made a home visit to interview the RA. RA would only talk for a few minutes, but he kept telling Treva that he was fine, and he did not need any assistance from DHSS. Treva was able to explain to the RA that the concern was that he would go into a diabetic coma, but the RA ended the conversation by closing the door. Treva left her card at the door along with resources on diabetes. EREPORT # 19106 stated the following: Leigh Ann Mathis has 4 Children she has lost custody of all of them. She is not allowed contact with Preston. I know this because the Nurses at the Hospital told her to leave or they would call Police. She is now living with Kyle and Preston. She was seen with Kyle and Preston at Christmas. Although being seen at regular intervals by social workers and health professionals there were many people and organisations that let him down. There was an obvious high level of concern for the childs welfare but the actions seemed to lack urgency and thoroughness. Joniya was seen unsupervised walking towards the WIC office in which she had to cross a busy street. Joniya had no shoes on and her family was not aware that she had left the home until notified by DHS staff. DHS staff was informed by another DHS staff member that the child was seen walking from the home. The mother stated that she was asleep and that her other children were supposed to be watching the child. This family has been involved with DHS in the past and has had an open case. Juliana has been attending school however, spends much of the school day in the office because she is unable to stay in the classroom for greater than 30 minutes. On 5/5/2017, I was called by Ms. Kathleen Seward, Principal at the West Somerville Neighborhood School. Ms. Seward reported she called the Crisis Team to come to the school as Juliana was "out of control" and had attempted to lock herself in a locker, and when approached assaulted a staff member. In the instance of intervening with the systems provided above a social worker could have been an advocate in a police report on Francine’s behalf. They could have provided officers with documentation on the severity of the probability of him injuring one of the children during his tantrums. The reporting party (RP) stated on 3/21/17 foster child Marian Mancilla disclosed to her classmate who she sustained a black (which eye not disclosed). According to Marian she injured her eye when she ran into her brother's shoulder. However on the morning of 3/20/17 Marian arrived to school with her foster mother Argelina who disclosed the eye injury occurred at the school on Friday. Subsequently, after the foster mother departed, Marian disclosed she ran into a pole near the bathroom. The RP stated she was present on Friday and Marian did not complain of an injury. PROCEDURAL HISTORY- Young stab victim was brought into the hospital on March 1981, who had no vital signs. The ER doctors on staff tried to resuscitate. He started to gasp and they installed a Emerson pump into chest to help drain air and blood from chest. 20 minutes his vital signs were back to normal. DR. Gerdes wanted to do a thoracotomy but was unqualified to do so. They then called a DR. who would be able to perform the surgery, Dr. Mc Cool. Dr. Gerdes explained to Dr. Mc cool that the patient was in the hospital and required a thoracotomy, but patient did not have insurance. Dr. Mc Cool wanted the patient to be transferred to Earl K Long Hospital to get better care. The Dr. explained to Dr. Mc Cool that he could not transfer patient and Dr Mc Cool said transfer him. Phone call was then ended and the staff had to make a decision on what to do. The Dr and the nurse were not happy about having to transport this patient. They called for the ambulance. When the paramedics showed up they felt uncomfortable in transporting this type of patient. They had to remove the Emerson pump, when doing so patient started to bleed. Vital signs went down and he died of cardiac arrest.…

After reviewing the case study of Nursing Malpractice in a Mental Health Setting. There are many different factors that had led to the unfortunate death of this child. I would begin with inaccurate documentation of this child severe airway disease, as being listed as mild reactive airway disease. Later, after developing symptoms of an increased cough and wheezing without the presents of a fever, the nursing staff dismissed it as his attempt to avoid group therapy. The next situation that is concerning is that he had requested to be taken to the emergency room for treatment and made a statement of “You’re going to kill me” and was told by the nurse “that he doesn’t need further treatment” and had referred to his outburst as attention seeking…
