Outcome Based Practice Essay
Goals and targets need to be set to enable outcome based care packages to be tailored to each individual. It involves putting the individual at the centre of every service required and not using a generic template. One size fits all has no place in the modern care systems. Instead at looking at the whole requirements it may be more prudent to break it into smaller pieces to ensure a successful outcome. It is ensuring holistically based approach is used to meet the needs of each individual.
There are three dimensions which have been put forward.
These being
Outcomes involving change, by focusing on developing self confidence or skills which empower self care.
Outcomes which maintain quality of life:- maintenance outcomes.
Outcomes involving the process of receiving services, or involving feeling valued and listened to.

I have received referrals for clients wanting to change providers due to their assessed needs not being met or assessed and a care plan implemented not right for the individual.
