Patient Raw Case Summary
This i s 4 year old AAF. Patient rwas seen 1/11/2016 for intermittent migraine headache, depression, anxiety, and insomnia. Patient reports since start of amitriptyline 25 mg QHS, Zoloft 50 mg QD, she feels much better, no depressive moods, sleeps well, and hasn't had any migraine headaches. Patient denies chest pain, SOB, N/V/D, or fever .Labs offered this visit, patient refused.
