Peer Evaluation Form For Z302
Z302 Peer Evaluation Fall, 2014
Your Name: _____________________________
Your Team Number: ________
Your Section (please circle one): TTH@2:30 TTH@5:30
Background & Directions:
As noted at the start of the term, each Z302 student is to complete a evaluation for each member of his/her team. The evaluation should be based on members’ commitment to his/her assigned team throughout the course of the term -- specifically regarding the level of contribution of each member. All evaluations are confidential.
All peer evaluations are due IN CLASS IN A SEALED ENVELOPE OR FOLDED. We can’t accept electronic submissions or paper evaluations outside of class (my fault, I may lose them, stick with me). Anyone not turning in a peer evaluation form will be seen as a bad teammate.
Don’t rate yourself. This form is all you need and asks that you: (1) Complete an assessment for each of your team members and (2) Designate the top performers on your team.
I. Assessment for Each Member: (PLEASE DO NOT RATE YOURSELF)
On the following 5-point scale, rate each team member on a set of high-performance team member behaviors:
5 – Strongly Agree 4 – Agree 3 - Neutral 2 - Disagree 1 - Strongly Disagree
Team Member #1 Name:

When forming teams in a professional work environment it is important to understand the personalities of your teammates to determine strengths and weaknesses. When a leader is able to define the strengths and weaknesses of the team members it is easier to delegate tasks, encourage brain storming sessions and be successful as a team. Team C is composed of five members all with great leadership skills and a desire to learn and complete excellent work together. The first week of class we completed a DISC assessment that identified our individual personality traits. According to "Disc Profile" (n.d.), "The initial DISC model comes from Dr. William Marston, a professor at Columbia University in the 1920s, who was curious about the behavior of normal people. He did not create an instrument from his theory, but others did” (Disc Overview). As a potential leader of this team, I was able to examine my own leadership styles and apply that to our assignment which is to create a professional development plan to identify the characteristics of the members to lead them to success.…

To conduct the 360-degree feedback, firms need to use evaluations from different portions of that involved in their business. First of all, it is important to collect assessments of an employee from his peers. In this part, employees are required to review for each other that are at the same level. Employees who are co-workers know each other’s ability better than anyone else from different levels in their company. In order to get reliable evaluations, peer reviews require all participants to be objective to others. By conducting this peer-to-peer employee performance evaluation, employees will not only enhance their self-development from peer reviews, but also learn how to give objective assessments to their peers.
