The overarching of the literature search and synthesis was identifying and critically appraising available evidence to support the development of the proposed EBP. The main strength of this review is the strength of evidence included. Six systematic reviews of RCTs, seven RCTs, and three quasi-experimental studies were included. Systematic reviews are were considered as the strongest level of evidence because they provide an unbiased evaluation of the existing body of knowledge relating to a particular practice, therefore providing the best and most reliable evidence. The setting is ABC hospital which is an outpatient health care center located in the metropolitan area in the State of Maryland. The care center provides comprehensive health care to all patients regardless of their race, religion, ethnicity, culture, or health insurance status. The practitioners in the hospital are a team of three registered nurses, one advanced nurse practitioner, three medical assistants, one registered nurse care manager, and two office receptionists all working on a full-time basis.
The hospital serves about 80 patients on a daily basis, and most of the patients have a diagnosis of various forms of chronic diseases including HTN, myocardial infarction, heart failure, diabetes, asthma, and obesity. Most of the patients have Medicare, Medicaid or private insurance programs, while a few have no form of health insurance. The hospital encourages the surrounding community to make the care center their home-health of choice. The values for the outpatient clinic are professionalism, integrity, provision of quality care, empathy, and performance improvement to accomplish the hospital’s vision and mission. The health care agency aims at improving the overall well-being of the surrounding community by providing patient-centered care. Therefore, provision of patient-tailored HBPM educational program is in line with the health care centers goals and
