helped since he is really cooped up at home. She said there is nothing cognitively that would keep him from driving. So we will be getting an appointment as soon as possible with PCP Dr. Malloy to get a release to drive. Mr. Johnson said he has not received anything from the State of Michigan. She said for him to come back in a year if he feels he needs to.
On 3/1/17 I met Mr.
Johnson at the appointment with Dr. Malloy. Mr. Johnson has requested my help making some appointment as he reports he is over whelmed. Mr. Johnson wants to resume driving. He was discharged from Covenant hospital with the instructions of no driving until cleared by physician. Dr. Malloy wanted Mr. Johnson to have a neuropsychology testing done before he would determine a driving status. This has been done. Dr. West neuropsychologist said she did not see anything that would say he could not drive but we needed Dr. Malloy to determine that. Mr. Johnson continues with physical therapy for his hips, and shoulder. He had a prior history of a shoulder injury and had hip pain. It was made worse by the accident. Mr. Johnson said the shoulder and hips are getting better. He has a home exercise program that he is following also. We had a long wait today to be seen. Mr. Johnson was also seen by a medical resident before we were seen by Dr Malloy. He was also seen by an intake nurse for some lengthy evaluations and history. Mr. Johnson reports frequent urination and blurred vision since the accident. He has an appointment for an eye exam and also with a urologist. Dr. Malloy said he did not see any reason why Mr. Johnson couldn’t drive. Dr. Malloy said he wants to see Mr. Johnson again in six months for a routine exam. He needs to finish up therapy and see the eye doctor and
urologist.

Mr. Johnson is a disabled Veteran.
ASSESSMENT
Mr. Johnson has started physical therapy and feels it is helping him. He has been released to drive. Once we finish with urology and the eye exam we will have completed treatment related to the auto accident..
PLAN/RECOMMENDATIONS
1. Contact Mr. Johnson every 2 to 4 weeks for update on his progress, pain and any new problems.
2. Attend the next appointment with Dr. Wang Urologist on 3/15/17, to clarify that current urinary urgency is not related to the auto accident.
3. Attend the appointment with VA Eye doctor on 4/5/17. Determine if blurred vision is related to the auto accident.
4. Contact Covenant physical therapy for an update on physical therapy progress and projections for completion.
