Quality Improvement In Health Care Essay
1. The stakeholders who are involved with the improvement in quality and safety are the board of directors and the executive and senior leaders. Drivers involved are families, providers and department mangers and directors. Others who should have been mentioned are patients, employees, regulatory agencies, accrediting and certifying organizations. Patients are vital to the success of quality and safety improvement because they can share their experiences as a patient in the facility, which can be used in ensuring quality and safe care is provided. Employees must be involved in the planning and implementation of quality initiative. They also must receive proper training on quality and safety to apply those practices in the workplace. Regulators, accrediting and certifying organizations also should be involve with the improvement in quality and safety to ensure the facility is in compliance with standards and requirements and (that) their initiatives comply with …show more content…
To overcome the barriers mentioned health care providers must form an attitude and acquire the skills that are necessary to implement quality improvement in healthcare. Once employees possess this, they are able to use the three steps discussed on how to improve. In addition, change agents, people who are needed to change the results a system produces, if they are able to work together and help improve the system, then patients and customers will have a better experience. This also can be accomplished by following the three steps to improve. Another way to overcome the barriers mentioned is using the Plan Do Study Act cycle to determine which strategy will work best to improve quality and patient safety areas in health care and then implementing those changes in practices and then measuring how they do by adopting, adapting, or discarding the change. Lastly, people are resistance to change and this barrier can be overcame by using a technique called motivational interviewing with quality

    A patient presented to the Emergency Department with the complaints of hip and leg pain. The patient rated the pain 10/10 on the standard pain scale. His (L) leg appeared shortened with swelling, ecchymosis, and limited range of motion. The leg was stabilized and then he was further evaluated and discharged to a room in the nursing department. The patient was also noted to have a history of impaired glucose tolerance and prostate cancer. The patient’s current medications were atorvastatin and oxycodone for chronic back pain. The patient was placed in a room and prepared for a procedure. The physician evaluated the patient and proceeded to order Valium, when unsuccessful hydromorphone was ordered. The patient had not achieved appropriate sedation for the procedure and additional medication was ordered. The patient was not placed on a cardiac monitor and a baseline oxygen level was not obtained prior to the administration of sedatives. The patient was receiving “Conscious sedation” in order for the physician to perform a manipulative procedure. The patient eventually had a decrease in oxygen saturation and became hypotensive- an arrest occurred. The patient was resuscitated and then transferred to a tertiary center.
