During a night shift in ICU, a 65 years old unconscious patient had massive bleeding from his IV catheter because I did not fix the end cap properly after starting the IV line.

Objective Recall Direct supervisor, resident physician and I were involved in this event. I fixed an 18 gauge IV catheter on the brachial vein of the patient’s right hand but I did not do the procedure properly. I put the end cap on the Luer lock but not accurately and I left the patient without double checking the IV line. I could not imagine that what happened to the patient if the supervisor did not come to check him. When I saw the bleeding, I felt extremely upset and asked myself about the cause of this error? Thinking about the fact that the patient was not conscious and he could not complain about this situation hurt my heart. I remember that my feeling was a mixture of confusion, anger and impotence. Since others considered me as an expert and responsible nurse, they did not judge me based on a mistake and I remember that they just had empathetic manner. Professional competence, correctness and sense of humanity were my values in relation to this event which were part of my professional values as a nurse. Despite the fact that I had enough knowledge and experience, I did not pay attention to the outcomes of this nursing procedure and put the patient at risk. I did not notice that how important was it to double check and monitor the patient after doing a procedure and what could happen to the patient if I did not consider the possible risks. According to Brennan et al. (2004), most of adverse events which occur in nursing procedures are related to their negligence. Motowidlo et al. (1986) state that occupational stress and job conditions influence nurse’s job performance. These literatures and my experience show that even if I knew how to do a procedure and I did it frequently, I needed to be aware of possible errors which happen due to such factors. I had to learn about these factors and how to manage them. Hence, I could prevent the problem and perform safe
