My Head To Toe Assessment

After spending one month in the female medical ward it was finally time to rotate to another ward. I was placed in the female surgical ward which barely had patients and was extremely quiet that day. The objective for that week was health assessment and physical examination so, therefore, I had to do a head to toe assessment on one of the patients under the supervision of a registered nurse. My patient was a forty-year-old woman who had a foot fungal infection called tinea pedis which caused her feet to swell, therefore she was unable to walk. Before I began the head to toe assessment on my patient I provided privacy, inspected her while I introduced myself and informed her what I am about to do. First, I took her temperature then I started with her head looking to see if there was any form of redness, swelling, and bawl spots. After that, with the use of my pen light I checked her eyes for pupil reflex and dryness, her nose and ears for any form of fluid and infection and mouth to see if there were any abnormalities. I also palpate her ears to feel for any tenderness and looked to see if they were symmetric and are the same colour with her face then I felt for lymph nodes under her neck.
Secondly, I checked to see if her breast was symmetric then I performed a breast examination. Since it was my first time performing a breast exam I was a bit nervous so it reflected on how I did the breast exam which was not a good job so my preceptor then showed me how to do it the proper way.
