In this stage, I can say that I had been person centered. The first instance was when I performed hand hygiene prior to contact with the patient, Susie Lee, because I wanted to ensure her safety by preventing the spread of infection. I was able to introduce myself as the nurse-‐on-‐ duty, but I noticed I did not really explain the reason for my visit, which is to assess the patient and perform morning care. I should have provided explanation in order to reduce client’s anxiety and promote cooperation. However, I feel it was inappropriate when I bent my knees when I asked Mrs. Lee to identify herself. Watching back the video, I realized that I was not keeping my eye contact to Mrs. Lee but I was side glancing to the camera once in a while. When I asked for Mrs. Lee’s relationship to Susie, I feel that the tone of my voice as an air of assumption. If I will rephrase it, it would have been “Hi, I’m Alistair the Nurse-‐on-‐duty for Susie today. May I know your relationship
