Case1
1. What happened to Claire Lewis?
She had a surgery to remove tumor at Hamilton General Hospital. Then she was transferred to McMaster University Medical Center to recover. However, that time to transfer her between hospital, her medical records did not go with her. MUMC`s staff did not know what drug she had gotten and how much, she was misdiagnosed with diabetes insipidus. As a result, her brain stem was crushed by excessive fluid and she was brain dead.
2. What factors led up to Claire`s death?
When she transfers to MUMC, her records did not go with her and MUMC`s misdiagnosis kill her.
3. What role did poor documentation or poor record-keeping play in Claire`s death?
When a patient is transfer to other hospital, the patient`s records have to transfer with patient. I do not know whether that is poor documentation, but HGH did not send her record. And it is the factor leading up to her death.
4. Could Claire`s death have been prevented? If so, how?
I think that definitely she could alive. She got successful surgery to remove the tumor at HGH, she transferred to MUMC to recover, but she died next 46 hours because of not sending her records and poor decision. The reason why I think she could live is that although her records did not go with her, before doctor diagnose to inject DDAVP, the doctor was able to check whether it is right. And if the doctor did check, think again, call to HGH to get information about her treatment or require her prescription, that kind of death would not happen.
5. How did the hospital respond to the situation?
Hamilton Health Science Chief of Staff Dr. Andrew McCallum offered written six-page apology with chief nursing officer Margaret Keatings and chief executive Murray Martin that is included about their responsibility and new culture of disclosure in health care.
6. How did the family respond to the situation?
The family did not try to get hospital`s apology willingly.

    Frank Gagnon Primary Care

    • 505 Words
    • 3 Pages

The place where Mr. Gagnon had his blood drawn was different from the place where he would undergo the procedure. When the couple finally arrived at their last destination (the main campus), the records were obviously not transferred properly as Mr. Gagnon had to fill the staff in on information that he himself was probably unclear about! Not only that, but Frank was not informed that he would be classified as an inpatient admission for this procedure, and so would need to stay for a longer period of time – neither were subsidized accommodations provided to him, by the hospital or his insurance company. His wife should have been allowed to stay with him on the hospital premises, since they did not have any nearby relatives and the wife was uncomfortable driving in a new city alone. His maintenance checkups will be back in Portland, for which he will need help coordinating transportation for, but the hospital seems to think the responsibility rests solely on his…

On the night of January 11, 1983, Nancy Cruzan lost control of her vehicle while traveling down Elm Road in Jasper County, Missouri. Cruzan’s vehicle had overturned, ejecting her from it, and Cruzan was discovered lying face down in a ditch. At the time of discovery, she was found to have no pulse or respirations. However, paramedics were able to restore her breathing and heartbeat at the accident site. She was then transported to the hospital in an unconscious state. The attending Neurologist diagnosed Cruzan of sustaining probable cerebral contusions stating that her upper brain, or the cerebral hemispheres, were too damaged to function. The part of the brain that controls thinking and feeling, the ability to move purposefully, compounded by a significant lack of oxygen, estimating that she was deprived of oxygen from between 12 to 14 minutes. After 3 weeks in a coma she progressed into an unconscious state, but in order to provide nutrition and fluids, with the written permission of her then husband, surgeons implanted a gastrostomy feeding and hydration tube. All other rehabilitative services were unsuccessful, Cruzan was in a vegetative state and the decision to move her to a Missouri State hospital was made with Missouri covering the cost of her care after her insurance run out. Nancy’s husband had their marriage dissolved by a court, and her…
