“Implement” “the Nurse Bedside Shift Report Strategy the Nurse Bedside Shift Report strategy is flexible and adaptable to each hospital’s environment and culture. As such, this guidance Provide choices and questions for hospital leaders on how to implementation this strategy .It may be helpful to Implement. These strategies initially on a small scale e.g. a, single unit single Identify lessons learned from the single-unit pilot “Implement” refine your approach, and then spread it to more units. In this way, you can build on your success in this as pathway to broader dissemination and wider scale change... Abstract the purpose of bedside nursing report is to communicate critical information pertinent to patient care. This transfer of information…
Firstly, the nurse must consider the patients demographic, body status and situation. Alice, a 54 year old woman, has presented with an ischaemic stroke with left hemiparesis- weakness of her left side- and partial visual impairment. It is essential for a nurse to have a clear understanding of the handover material as it gives them vital information about the patient and aids them in making decisions about how to proceed next. The nurse must note…
Our organization is focused on providing compassionate, high quality, cost-effective services in a safe, efficient, and effective manner. In 2012, the Joint Commission implemented a new tool for hand- off communication to help organizations avoid errors and miscues during the passing of important patient information from one individual to another (Joint Commission, 2012). This tool can be incorporated into our project by examining our current handoff process, provides a measurement systems, and assists with guidelines to increase effective communication within our department. In order for our department to be more aligned with both our core values and the recommendations of the Joint commission we must adapt to the following four key outcomes…
The researchers concede to this and state that there is little empirical evidence constituting what is best nursing handoff practice (Riesenburg et al., 2010). “The Joint Commission is calling for structured handoffs; yet we found very little evidence to support the use of any specific structure, protocol, or method.” (Riesenburh et al., 2010, p. 32). Consequently, the researchers succeeded in observing that more high-quality research must be conducted in the pursuit of evidence-based practice. Rigorous outcome studies are needed that focus on and “assess the effectiveness of nursing handoffs, determine which elements lead to improved outcomes, and identify the best implementation strategies” (Riesenburg et al., 2010, p. 32). The compiled barriers and strategies in this review for effective nursing handoff could easily be sourced or utilized as a roadmap to an upcoming research study. However, if the researchers had cast a wider net with their search, such as including unpublished articles, different databases, and non-English publications, maybe the results would be different- more succinct or even more varied. It is hard to…

Collection, analyzing, and interpretation of data are important functions at the skilled facility. The nurses are major contributors of data that is either, generated electronically or by the health record department staff. Health care provision today is centered on evidence-based care, which continues to influence the type of care patients receive. Statistical information enables the health care providers to plan, formulate current and future policies, improve patient care through evidence-based studies, evaluate, and improve on customer care, and for compliance purposes. Examples of data collected on daily basis are the daily patient census that summarizes the total number of patients per unit by the end of the day computed manually at 12 midnight. This reporting tool is presented in tabular format and shows all new admissions, readmissions, in-house transfers, transfers out to other levels of care, discharges, deaths, or missing patients. Another example of statistical…

Mr Johan experiences symptoms of panic attack when he feels self-conscious in front of other people when performing certain tasks. He reported feeling faint and had black out of thoughts during those episodes, whereby he was not able to think of anything. Both his hands would tremble and become numb. He would also be sweating but experienced no symptoms of pounding heart or choking sensations. Those panic symptoms would often last for one to two hours or until he removed himself from the anxiety provoking situation.…

The purpose of this of this experimental study “Investigate transfer of information at nursing handover” (Boyd, Cumin, Devcich, & Lee, 2015, p. 161) is clear and concise. The dependent variable of this study is the nurse’s confidence in the truth of information and independent variables is the way the information is presented. The sample population is clearly defined, and the result “ adds to the literature because it is one of very few studies done using an experimental design to investigate nursing handover” (Boyd et al., 2015, p. 165).…
The Standardized Language of the nursing practice (SNL) is an effective communication process that delimits professional nursing practice (Rutherford, 2008). The standardized language made contributions for the current care to another nurses, health care providers, and others members of the multidisciplinary health care team (Rutherford, 2008). This language was standardized with a purpose of the nursing documentation to straight the nursing care (Rutherford, 2008). According to the American Nurses Association (ANA) (2008), has been accepted thirteen standardized languages that support nursing practice. The contributions of the SNL are better communication…
I didn’t find any articles mentioning the association between the bedside handover and the patient length of stay. Nevertheless, I found two studies suggested that bedside handover helps the nurses to effectively discharge the patients reducing the chances of delayed discharge (Wendy Chaboyer et al., 2009; Wendy Chaboyer, McMurray, & Wallis, 2010). Therefore, I believe that changing to bedside handover practice can help to improve the work efficiency of the nurses resulted in faster discharged rate and decreasing patients’ length of stay.…
Preregistration was created in an effort to decrease receptionist and billing office workload and patient waiting time on arrival to the clinic. If only 16% of the registrations are completed upon arrival, then patients will have longer wait times as receptionists are gathering demographics and billing offices are completing insurance authorizations. This incomplete registration process creates an escalating effect as nursing staff remains delayed all day and must perform service recovery with patients for the delays. Appointments are generally late and providers sense that patients are frustrated before the physician can begin care. Because staff are rushing through appointments, the billing office has identified that care is given that cannot be reimbursed due to incomplete records. In reviewing each stakeholders’ point of view, preregistration is one large problem that needs process improvement. The goal of this performance improvement project is to streamline the registration process in an effort to decrease wait times upon arrival to the clinic. Removal of wait times will have an inverse relationship with patient satisfaction, thus as wait time decrease, satisfaction scores should…
