In my personal career, it is very important for me as a nurse to advocate for my patient and communicate his or her needs. I must do this by articulating those needs to the hospice medical doctor, social worker, chaplain and nurse aide. This will allow our team to ensure the comfortability of the patient and surrounding support. In the world of hospice nursing, I have the responsibility and moral judgement to make certain my patient maintains a level of comfortability. My collaboration with the social worker is significant because her efforts are needed to navigate the everyday and emotional matters that arise through the patient end of life journey, correspondingly the chaplain partnership is needed to help the patient identify spiritual concerns and assist in bereavement services to the deceased families. With the collaborative efforts of all teams, this leads to not only optimum care to the patient but optimum care to the families as
