301 – Set objectives and provide support for team members

Describe the purpose of your team:-
The purpose of my team within my setting is to ensure the running of the business and to make certain that the service individual needs are met to the highest standard. You have to adhere to all the policies and procedures.
Within my setting all team member are interviewed and have to go through a lengthy to ensure that they will fit in with the team and be able to meet the service user’s needs.
A good team have to get along with each other and work together so that the best outcome for the service individual is met.
As a team we aim to protect our service individual from abuse, neglect to prevent deterioration of or promote physical or mental health, to promote independence and social inclusion, to improve opportunities and life chances, to strengthen families and to protect human rights in relation to individual social needs.

How have you identified opportunities and difficulties faced by team members in achieving their objectives? How did you discuss this with them and what advice and support did you provide to overcome the challenges.
When I was doing supervision with my team leader for one of my colleagues, one thing I brought up was the medication, as there seem to be issues when doing this especially doing a medication count. Whenever they had done a count or signed them off they seemed to be making mistakes, this had happened on numerous occasions and medication assessments had been done a number of times.
We did an action plan, and after speaking to the person it came apparent that it was panic and lack of concentration when doing this. I encouraged them to do this in a quiet room away from any distraction, they watched me a few times doing this and then they did the medication whilst I supervised on a few occasions, medication assessments were completed and it wasn’t until the team member felt confident that they were left alone to do

Distracted ED staff without the proper skill sets may lead to delays or errors in diagnosis and treatment, including delayed or omitted laboratory testing, procedures, and medication administration. In a study conducted by Penska, communication errors were the root cause in about 70% of cases when sentinel events occur (Pesanka, et al., 2009). Medication safety is a critical area to consider when examining process measures related to boarded ED patients. The risk of medication errors in ill boarded ED patients can lead to poor patient outcomes such as a decrease in blood pressure and oxygenation, heart arrhythmias, and multi-organ dysfunction (Richardson & Mountain, 2009).
