According to the e-mail Mr. Anderson fell on 5/25/16 at approximately 11:00AM and sustained a laceration above his right eyebrow while ambulating. At 3PM the same day the Mr. Anderson seemed incoherent and his speech was slurred. The offender is in clinic today after she had an apparent seizure on 12/31 in the evening. She had an ICS activated at that time. It is not clear exactly what she was doing but she fell onto the floor and scraped the side of her face on the right side abrading it on the rug or something. Since then she has been putting bacitracin on the area. She says that she missed a couple doses of Keppra prior to the incident. She says she was not trying to skip the medication or get off of it she simply forgot to take it. She normally takes 500 mg twice a day. She has had a seizure disorder since birth. Her last seizure was when she was in Waseca at the Federal Program. She was returned here due to her medical complications including her epilepsy and thyroid condition. She has been hyperthyroid. She was last seen for that in early December and she has a repeat visit coming up soon for that disorder. She denies any pain or numbness, weakness, or tingling currently. She had complained of a headache the evening after the seizure but has been doing well otherwise. She is in school and she says she took a sick today because of her face.…

At 4am dispatch received a call from the resident at 137 West Alexander Drive, who claimed to have heard a scream from his neighbours house. Captain Jason Long turned the case over to Officer Brice Tyler, Officer Lily Rayne, and I. At approximately 4:30am we arrived at the house knocking on the door. The young man answered with a smile on his face. I had noted that he seemed wide awake, as if he had been up for some time, as he invited us inside. We told him of our cause of being there, and he immediately told us the old man was visiting his sister in another town. He did not give us more details. instead, he showed us that the old mans belongings were exactly in there proper place. Afterward, he asked us to sit down in the bedroom, chatting easily about his evening. Soon after, he seemed to become agitated, raising his voice and speaking angrily. It was sometime late at night on May 28, 1971 when Donald Marshall Junior and Sandy Seale were walking through Wentworth Park. During their walk, they met up with two other gentlemen named Roy Ebsary and James MacNeil. (1) At this time they had engaged in a conversation with the two men. At some time during their conversation, Donald and Sandy attempted to pan handle them. However, Mr. Ebsary was intoxicated and didn’t take very kindly to people of a different race. He shouted something to do with being black at Mr. Seale then proceeded to stab him in the stomach. After he stabbed Sandy he swung his knife at Donald. He cut Donald's arm. On Thursday September 14th at around 12:30 pm the hospital had a sentinel event which involved a missing child. The event details are as follows: Tina was scheduled for same day surgery and was brought to the hospital by her Mother. Her Mother was informed that the surgery would take approximately 45 minutes and that Tina would be in recovery at least 1 hour. Tina’s Mother informed the pre-op nurse that she had an errand to run involving Tina’s older sibling. Tina’s mother left the pre-op nurse her cell phone number with instructions to call her if Tina got out of surgery and was ready to go before she returned. Prior to Tina’s mother returning Tina’s father showed up to check on her, and finding that she was ready to go he took her to his house until the mother could pick her up. When Tina’s mother returned 2 ½ hours later she found Tina missing and the staff did not know that her father took her. The hospital activated a Code Pink and notified the local police department. When the hospital security interview the mother they learned the parents were divorced and that she had full custody of Tina and her other siblings. The local police found Tina at her father’s home within 30 minutes of Tina’s mother returning to the hospital. No charges were filed against the father.…
Working with the information provided by the informant, Sergeant Crouch, sent an email to Voyles describing himself as a fifteen year old girl named “C.J. Best” on December 28, 2000. In this email “C.J. Best” told Voyless that she was looking for a new chat buddy and that she had gotten his address through some friends and that she lived in Forth Worth, Texas. About two and a half hours later Voyles responded to the email indicating that he was interested in being chat buddies with “C.J. Best.” On January 19, 2001, after several emails were exchanged between Voyles and “Best” Sergeant Crouch obtained search warrants for Voyles home and work computers where the Arlington Police Department found and “seized child pornography off the hard drives of each unit.” Voyles was charged and indicted for possession of child pornography which is a third degree felony.…
