Organizational structure and systems work hand in hand, a structure could be good but is not efficient on its own, and a management system gives a solid foundation of running an organization and implementing changes. Management systems include the organizational relationships, performance, and competition, all stakeholders must be considered in designing the systems (Tiller, 2011). Change may be perceived to have negative economic implications, change of organizational structure and system may result in laying off, demoting or reducing of salaries. The staff tends to resist change if they perceive it to have negative economic implications. The threat to power and influence is also another factor that raises conflicts and resistance to change. Ellen Zane facilitated a shift in the institution's strategy, redistributed power, changed the structure and control system. There was a major break and a major reconstruction of the organization took place. When Ellen Zane joined Tufts-NEMC she started implementing change, she appreciated the senior staff expertise and experience but she felt that she needed some needed replacing, seven members of the senior management team were replaced. She was able to be fully transparent with the staff as she created an efficient communication platform which included meetings and emails. She initiated the agenda of change which involved cutting costs and making operations efficient. She saw that the length of stay was reduced, that the institution got higher rates from the major health plans, she built networks and brought back affiliations tat were lost and she reinstated the relationship between the Hospital and Tufts
