After the client provides their registration information, I would start with questions in order: the client family history, socio-economic history, developmental history, emotional/psychiatric, medical history, substance use history, and then the symptom checklist. On page two, I would condense the section containing the symptoms to eliminate the repetitiveness and provide simple terms. Because this intake form is organized in clusters, it does not provide much space for documenting information. I would space out the sections providing more space for the clients to record their information. This might create more pages but all the information would be able to be addressed.…
The treatment I would recommend Mr. X is cognitive-behavioral therapy. According to Beck most cognitive-behavioral therapy has been of the two best-known psychotherapies for unipolar depression (Beck). Cognitive-behavioral therapy will help Mr. X deal with his current problems. Cognitive-behavioral therapy is a good way to help Mr. X evaluate how accurate his dysfunctional and negative beliefs of himself are. Cognitive-behavioral therapy will also uncover where his pessimistic beliefs are deriving from and how to change those beliefs. In addition, I would also like Mr. X to attend family and marital therapy. It appears that his symptoms had been present for years. On August 8, 2003 the Illinois Children’s Mental Health Act (ICMHA) became law (LuAllen, Koch, Taylor, Payton, Weissberg & O’Brien, 2005). A report by the Illinois Children’s Mental Health Task Force (2003) states the current system, prior to passage of the ICMH, is “fragmented, limited in scope, and poorly resourced”. The Illinois General Assembly is attempting to address these issues with the passage of this act.
