Mr. Lee, who is a living alone elderly with nutritional concerns, was referred to the community center for free lunch. He showed chewing difficulties resulted in imbalance food intake. The report is hoped to improve the client’s health and living quality through alleviating his nutritional problems by providing some effective and simple ways for him to follow. The following is structured into several sections. It will first focus on analyzing the underlying diet problems of Mr. Lee, then a few further assessments will be recommended. Finally, suggestions will be provided for storing food choice and the nursing interventions including diverse aspects of patient’s demand.

Intervention (i) Nutritional
On Mr. Lee’s diet, the kitchen should prepare a diversified meal which is suitable for elderly. We can calculate the daily protein and energy requirements for individuals. (Correia et al., 2014) The menu can add some soft food but rich in protein. For instance steam fish, egg or considering changing to a soft diet. It should include substantial amount of squashy vegetable to increase the dietary fiber. (Food Standards Agency, 2007) We are able to give a copy of the recipe to the client so that he can follow and maintain the improved nutritional state at home.
Routine checkup would be the key element in maintaining health. Starting frequent body assessment like BMI and IO chart, to provide us data about his nutritional status. By comparing with the baseline data, we are able to evaluate the effectiveness of intervention. Abrupt change in value can also alert us to initiate further investigations.
Mr. Lee is in shortage in energy, he can consider taking home fortification and oral nutritional supplements (ONS). For instance, skimmed milk powder for protein source. They are useful in provision in energy and protein. (Baldwin et al., 2015) In addition, he should be arranged in taking small frequent meal. For eating guideline, we will advise them to chew as much as possible before swallowing. For balance diet, we will teach them government’s health campaign such as the food pyramid and “2 Plus 3 a Day" healthy diet. (GovHK: Healthy Eating, 2014)Through teaching, it is believed that they will be able to recognize the benefit and execute it in everyday meal.
(ii) Psychological
Chewing disability may affect his psychiatric health and may even cause depression in elderly. (Laudisio et al., 2013)He may have a bad mood for incapability and reduced food choice. Therefore, we should carry out continuous monitoring on mental status such as watch out for signs of depression and isolation. We are encouraged to communicate with him frequently to give reassurance and build connection. (Hegerl, Rummel-Kluge, Värnik, Arensman & Koburger, 2013)This may reduce his anxiety and reduce the risk of mental disorder toward his problem.
(iii) Social aspects
We can arrange other elderly in the community center to carry out group discussion about healthy eating. Other elderly encountering similar situation can share topics like food suitable for him which are tender and nourishing.
