Dear Sir or Madam:

Mr. Lightman we received your voicemail stating that you wanted your records sent to another physician. We as an office would like to take this time to apologize for the way you were treated in our office on February 12, 2012. In your voicemail you stated concern about how you felt unwanted in our office and that our staff was rude. This is not how we like to treat or patients or how we conduct our office in anyway. I have personally spoke with our staff about this situation. I also to a look back at our schedule on the day you were here to try to figure out the reason if any you were treated the way you were. In my investigating I found that we has emergency situation in our clinic at the same time you were being seen. Although there is no excuse for the way you were treated this may be why our staff seemed distracted or rude. I want to tell you personally you are always welcome and wanted in our office. If you would still like your records sent to another physician please feel free to have that clinic contact our clinic and we can get that done for you. Again I would like to personally apologize. No matter what is going on in the clinic the staff should always treat every patient with the upmost respect. We hope to see you in our office again. Please feel free to contact us with any questions or concerns you still may have. Thank you for your time and being one of our patients. Have a great day.

On September 27, 2016, CPT Porter, Derek came in for his appointment; however, he did not have his ID card with him. At that moment, I informed him that I could not check him in unless he has his ID card. He replied, “Even if they already know who I am”, I said “yes, even if they know who you are, I cannot check you in without your ID it is protocol”. I proceeded to inform him that if he had another form of identification he can go to medical records and get a promissory note. At which time he became upset and stated he wanted to speak to someone. I immediately, asked Major Stackhouse for assistance, I explained to him the situation and also informed CPT Porter that he had to obtain a promissory note. In addition, Major Stackhouse offered to inform the provider of the situation.…

Being hired as a new administrator for Gotham City Medical Center which mainly focus on mental health issues comes with a host of task. The inpatient and outpatient services are housed in separate buildings on the same campus which caused problems by the center during file recording. After an audit conducted by the New York State Office of Mental Heath several findings were highlighted that concerned them. The two primary findings which were patients notes were written once a week which tells me that the therapist did not record any individual notes at all. The second finding was that sign-in sheets were present for those who attended but no notes on their daily progress and visits were recorded in their records. Also patient mental health information was shared with the media which is unethical. As a new administrator, I would be entering into a setting where unethical activities are occurring, overwhelmed staffs and improper handling of patient files. As a new administrator, the findings require attention to develop a system for providing better mental healthcare services.…
