Ati's Fundamentals Reflective Report
Over the weekend, I answered some NCLEX questions and studied for the upcoming test for the class. I went over the activity and immobility detailed course outline. I looked through the ATI’s Fundamentals of Nursing book to get a better understanding of what I am learning. I went over the steps on making a bed, taking a patient off a bed to a wheelchair, and providing care for the immobile. After that, I did some more NCLEX questions and did a little reading for head, neck, and neuro.
NCLEX questions:
1. Those wounds that are left open for 3-5 days to allow edema, infection, or exudate to drain?

The answer I chose for this question is “secondary healing”. I chose this answer because secondary because the wound is left open. The correct answer

Hello Dr. DeVoe and classmates, currently working on a surgical intensive care unit (SICU) for the past 7 years, before that worked on an open heart ICU for 16 years. I have been a nurse for over 30 years and started as an LPN and achieved my BSN in the year 2000. Have been a preceptor for many years but have no experience in curriculum development, so this will be a new journey for me. Live in Queens in NYC, care for my mother who has Alzheimer's disease. Have only one son and have an 18 year old granddaughter who is attending college, with a goal to be a nurse. Looking forward to learning from this class and…

I was sure to do my study guide and review it before clinical, as well as completing all labor and delivery drug cards, including two extra. This helped me understand what the nurses and doctors were taking about. This clinical in L&D, which enabled me to use the wave understandings that Traywick taught to us the previous week. This helped me see how the patients were doing in relation to contractions and how the baby was responding. This was interesting and neat to have an idea of what it means instead of going throughout clinical without any insight on this.…

The NCLEX test blue print will be a useful guide for me throughout nursing school. It provides me with the basic idea of how the test will be laid out and what kind of information will be included. Having read the first few pages, I find it very useful because it breaks down the nursing process and how to apply it to different areas in nursing. In addition, the nursing test blue print provides several scenarios, in which a patient is at risk for potential body alterations, whether it is psychosocial or physiological, and it explains the rationale of every answer.…

The third phase of wound healing is granulation phase, which takes place to repair the damaged cells by regenerating new cells. This phase consists of different subphases, which can last up to 4 weeks in the clean and uncontaminated wound. These sub phases do not happen in discrete time frames but constitute an overall and ongoing process. The sub phases are "fibroplasia, matrix deposition, angiogenesis and re-epithelialization" (Cho & Lo, 1998).…

There are three specific kinds of wound drainage. First is serous exudate, this is where the drainage consists of serum, a clear portion of the blood, it's clear and watery. An example of a serous exudate would be a fluid blister from a burn. Sanguineous exudate consists of red blood cells making it red in color. This occurs because capillaries are damaged, allowing the escape of red blood cells.…

I felt I had a grip on my time management. I was able to document right after my assessment with all my patients. I find that this not only helped me remember every detail of my assessment but it also allowed me to go back to the patient at that moment, if I missed a part of my assessment, and finish the documentation. My nurse this week taught me things that I had not learned in the previous semesters. I felt sure of my nursing skills and was thinking “like a nurse”.…

    1. To prepare for the NCLEX-RN, I plan to devote the months of May and June to studying. I intend to spend the majority of my time going back through my notes from all of the semesters and making note of things that I will need to refresh on again just prior to taking my NCLEX. As of now, I don’t feel as though this is something that may be unrealistic however, I understand once I get into actually studying my plan may change. I also have a Kaplan NCLEX book I plan to utilize to prepare for this exam, as well as using online resources such as U-World. As of this moment, I am unsure if I will be able to participate in the Hurst review but, I am still looking into my options.…

    Online Library Tour

My library tour at Texas Woman’s University had begun even before the semester kicked in. Not surprisingly, a former nursing student like me knew very well that the semester would become overwhelming very soon. An early jump-start getting to know some available resources would curtail my time spending later for all writing assignments or researches. Armed with previous library experience, I set out for my exploration by going directly into a “RESEARCH” tab and easily located the nursing field with abundant database sites. I carefully explored all the sites and I felt relieved because I had help on hand, and didn’t have to worry about being unable to seek help any more.…
