c. Anxiety related panic attacks

f. Feeling she must be perfect
g. Having high expectations of herself
h. Inability to eat (eating disorder)
i. Committing to things she cannot finish
j. Over stretching herself

Following the first interview, I tentatively identified the following as potential issues:
a. Anxiety related to social issues
b. Anxiety in most areas of her life
c. Panic attacks when her anxiety gets out of control
d. Self-isolation
e. Issues related to low self-esteem

Before the start of the next school semester, reach out to disability services to complete the paperwork for school accommodations to be put in place. These accommodations will decrease unwarranted stress and anxiety attacks.
C. Plans
1. Action Plan/Service Approach
To achieve the final goals, Mrs. Hungness and I agreed on the following action plan: Mrs. Elena Hungness and I (Polly Hewitt, social worker) will meet for three 1-hour sessions over a period of the next three weeks. Our purpose is to work together toward achievement of the final goals identified above. We will approach this work as a collaborative and cooperative effort, with each party contributing ideas and suggestions. Throughout the next three-week period, we will monitor and rate the degree of progress. At the end of our time together we will determine whether to conclude our work, consult with or refer to an outside source, or contract with each other for further work together.

She agreed she would write it down in her planner or set an alarm on her phone as a reminder. She will take it with food to avoid any unpleasant side-effects.
