Case Study: Orchard View Care Home
Orchard View Care Home was closed down in October 2011, 19 vulnerable adults died due to poor practices. A five-week inquest heard how some residents were given wrong doses of medication, and left soiled and unattended due to staff shortages. Call bells were also often not answered for long periods or could not be reached by elderly people living at the home.
Several examples of poor care and neglect were revealed during the inquest, including:
 A woman who had not stood for six months was pulled out of her chair to a standing position
 A dementia sufferer's broken ankle went undiagnosed for days
 Nurses shut doors on elderly people if they called for help
 An elderly woman was left in a toilet and forgotten

This sort of poor practices were carried by the service providers in Orchid View; the service providers were harming service users for example: a woman who hadn’t stood up for six months was pulled out of the chair to a standing position this situation is risk of harm to the service user. The practice carried by the service provider in Orchard View is unacceptable, as a service provider they should be more aware of harms to the service users.
Abuse and neglect
Actions that lead to harm to others. This can include physical harm, emotional harm, financial harm or sexual harm. Abuse is usually taken to mean deliberate acts or failures to act; inadvertent harm resulting from a failure to act appropriately is usually referred to as neglect (G. Brotherton and S. Parker 2013).

When a service user owns medications are no longer required they should be disposed of safely and always remember to keep the record of who returned them, the quantity and date. A witness to this transaction is good practice. It is service provider responsibilities to throw away any sharp objects and items, such as needles, in the proper way. Because needles and sharp objects may break through the red trash bags, they must be put into hard puncture proof containers (yellow box). These sharp items are also treated as hazardous
