The factor of safety is very important because a very low factor of safety may lead to fatal accidents, jeopardizing not only the lives of the pilots but also the personnel working on the flight deck. The recovery, investigative process as well as time and monetary costs can be extensive and as such are to be avoided. This explains the reason for a high factor of safety which is to minimize the percentage of failure.…
The British health and safety at work act 1974, was enforced to ensure that general duties in the work place were conducted safely and correctly from employers towards employees and members of the public, and the duties that employees have to themselves and to each other.…
Brace for impact are the last words one wants to hear while fling along an a aircraft, but that is exactly what flight attendants yelled to passengers aboard a US Airways’ flight in 2009. Whenever someone thinks about an aviation accidents they always seem horrific and have staggering death tolls. But when you stop and think about the fact that there are over 7,000 of planes in the sky Untied States at any given time carrying hundreds of passengers and crew really aviation is quite statically safe. (Federal Aviation Administration, 2009) In fact you have a better chance of getting hit by a lighting or drowning then dying in a plane crash. (Britt, 2005) The passengers and crew were given the “brace for impact” command by the plane 's captain and corresponding the aircraft accident would forever be known as the “Miracle on the Hudson” or ditching of United Fight 1594. I’m sure you can remember seeing to picture of a plane, with passengers standing on the wings, floating down Hudson River between New York and New…
position in terms of longitude and latitude. Without adequate warning of a problem, a pilot would be unaware that anything is wrong until it is too late. In past situations such as this, the ensuing crash is usually fatal. In a study done by Captain Dave Carbaugh of the Boeing Commercial Airplane Group titled "Avoiding-Controlled-Flight-Into-Terrain", he states " that in the days of propeller driven commercial aircraft; CFIT caused over half of the aviation accidents recorded." (Carbaugh, 1997, pg 98) He also states that since the late 1950s, the beginning of the jet age, more than 9,000 fatalities have been attributed worldwide to commercial aircraft inadvertently hitting the ground. These statistics prove that CFIT has been one of the leading causes of aviation disasters throughout history.…
In short, I strongly believe that airplane crashes happened because of the pilots awareness and unusual weather, but Hatry believes the accidents are happening because of the airplane designs and mechanical problem that happened to airplane engines.…
The stalled and started its descend to earth. They tried to restart the engines but they wouldn’t ignite. Therefore they crashed the plane behind a column of houses. The only fatalities were the two men on board. The cause of this accident was the pilot and co-pilot acting in an irresponsible manner for which was the cause of their fatal deaths.…
I’ve heard, that most airplane accidents take place during takeoff and landing. I’ve also heard that accidents occur more often because of pilot errors and less so because of mechanical problems.…
The Tenerife Disaster is a well known event among the aviation industry for being the worst air accident in human history. A Dutch KLM 747 and an American Pan Am 747 crashed into each other at a Spanish airport in the Canary Islands resulted in 583 death and a extensive media coverage on the Dutch, American and Spanish subjects. The disaster was investigated and studied by the International Air Transport Association (IATA), the International…
On June 1st, 1999 American Airlines flight 1420 experienced a tragic accident that claimed many lives and made an impact on aviation worldwide. The event and it subsequent investigation shed light on issues and pressures airline pilots face and resulted in new technologies and new regulation that have made aviation safer for all pilots and passengers. Any aviation accident that results in the death of pilots or passengers is a tragedy but these accidents present lessons to be learned through investigation of the human factors, industry efforts and regulation and other factors of aviation safety.…
In the US, each year there are about 40,000 deaths per year in automobile accidents vs. about 200 in air transport. To put this in perspective, the chance of dying in an automobile accident is about 1000 times more than winning a typical state lottery in a year. The statistics are based upon the number of airplane crashes as a percent, in other words more people die as a result of automobile crashes per year as opposed to dying by an airplane crash. The survival rate is much lower for an airplane crash but airplanes don't crash as much as cars.…
“If wisdom can be attained, one should not just acquire it but enjoy not to the full”. With wisdom comes justice because if you are wise then you can fully grasp the fact of justice. Justice is something Cicero took pride in finding the answer to what is Justice really? To Cicero being “just” isn't just having the right morals and treating people how people should be treated, but more as a lifestyle. Cicero’s definition of justice as stated int his book On Obligations he says, “The primary function of justice is to ensure that no one harms his neighbor unless he has himself been unjustly attacked.”. He also says in the book On Obligation that another foundation for justice is good faith and trusting in the things that they say to you, and…
Aloha Airlines Flight 243 Boeing 737-200 April 28,1988 Honolulu, HI Aloha 243 was a watershed accident : There were very clear precursors that were not acted upon It reflected a basic lack of airplane level awareness It pointed out errors in basic design and certification philosophies There were catastrophic unintended effects April 28, 1988, Flight 243 was scheduled for several “Island-hopping” flights: First Officer conducted preflight inspection in darkness and noted nothing unusual…
All engineering is based on risks and hazards. Nothing is perfect. A prototype still contains lots of errors. On lower level engineering projects, these may not be so harmful but on a higher level project they can be deadly. In 1986, the Space Shuttle Challenger exploded during launch, taking the lives of six astronauts and one teacher. Questions arose about what factors led to this disaster. I will discuss these factors in terms of general engineering disasters.…
