The difference between a case manager and an advanced practice nurse is the case manager deals with the patient and family more holistically. Case managers are aware of finical situations, living arrangements, and family support. When it comes to advance nursing practice those individual are also treating patients holistically but not on a one-to-one bass like case managers are. The further looks bright with care management nursing. Hospitals are utilization more out side recourses to help prevent re-hospitalizations. Services such as palliative care, respite, and home health are great examples. The on going education a home care nurse can stop multiple re-hospitalizations if used correctly. More and more patients being discharged from the hospitals who are going to rehabilitation centers are also being provided with case
