Risk for Hypertension related to inappropriate lifestyle practices
Subjective Cues:
“Pagbuntis nako sauna kay taas akong BP.”, as verbalized by the client.
“Taas pud ug BP akong bana.”, as verbalize by the client.
Diet is usually composed of salty food. She usually sleeps and talks to neighbors as a way of relieving stress.
Risk-prone health behavior related to deficient knowledge and economic difficulties
After 3 weeks of nursing intervention, the following are expected to take place:
The family will be able to initiate lifestyle changes that will permit adaptation to current life situations.
After 45 minutes of health teaching, the family will be able to:
Identify the risk factors of hypertension
Enumerate nonpharmacologic measures that minimize risk of hypertension
Identify improper practices that could worsen the family’s health condition
Assess the family level of understanding regarding hypertension
Discuss with the family the lifestyle practices needed to prevent hypertension such as:
Smoking cessation
Regular exercise
Limiting alcohol intake
Stress Management
Encourage to balance out the time for exercise, daily activities and rest by teaching the family time management.
Teach the family exercises that they could incorporate in their daily activities that positively affect their blood pressure.
Discuss with the family the diet & cheap foods that prevent hypertension such as:
- low in salt, fat, sugar (ex. buwad) high in potassium, magnesium, calcium, fiber, protein and Vit. D. (red rice, beans, banana, potato)
Discuss with the family the importance of regular check-ups and availability of free resources in the RHU.

The family will be able to practice proper nutrition and diet that lessen the risk of hypertension
The family will be able to:
Recognize the diet recommended for hypertensive individuals
Identify cheap but healthy foods that belong to this category that they could incorporate in the family’s diet.
The family will be able to demonstrate how to utilize resources in the area
The family will be able to:
Explain the importance of regular check – up in the RHU
Find ways to incorporate exercise into their daily activities
