Below is a diagram I made showing the analogy between a functional team and my team – the Post-Acute Team (PAT).
Each team in our department has a specific purpose and my team’s main function is to facilitate the smooth transition of members from hospitals to other levels of care including skilled nursing facilities (SNFs), long-term care facilities (LTCFs), hospices, and homes.
Below is an illustration I made showing the transition of care from hospital to different levels
of care.
As a functional team, there is a clear line of authority and everyone recognizes the chain of command.
For clinical or medical determination issues, we consult our Medical directors who report directly to the Chief Medical Officer who manages all the Medical Directors.
For our administrative issues, we, the team members, report to our team leader – the Assistant Director, who in turn reports to the Director of our department. Administrative issues that we discuss with our team manager include departmental guidelines and procedures, workflows and work processes, staff performances and staffing schedules, and organizational changes and communication.
Below is a diagram I made showing the chain of command for administrative issues Clinical or Medical determination issues include decisions involving authorizations, approvals, or denials needing reviews and analysis from the medical director.
