Gifted Facilitator Notice Case

Gifted Facilitator Notice of Meeting signed by mother and Jeremy. Jeremy is 18 years old, and is considered his own decision maker for this meeting. Both mom and Jeremy signed that Re-evaluation not needed and that special education services would end at the time of his graduation. The Prior Written Notice was also signed by mother and Jeremy reflecting the change in services. Both mom and Jeremy agreed that services would not be necessary once Jeremy entered college.

Parental Concerns: Mrs. Mettner asked if mother or Jeremy had any concerns. Both stated that they had no concerns.

Strengths: Mrs. Mettner stated that Jeremy has strong skills academically and socially. Jeremy’s SHS teachers have reported that they are pleased with his work. Jeremy is currently participating in an internship at the Kansas Lottery. The employees have reported that he is doing well. Jeremy plans to continue this internship through the summer at the Lottery.
He is set to go to Kansas State University in the Fall (2017). His course of study at KSU is Marketing.

Student Present Level of Academic Achievement and Functional Performance: Jeremy needs to take Advanced Placement and Honors class to be challenged. The team discussed Mr. Fries class and how much Jeremy appreciated him as an instructor.

Goal: Since the amount of time is very limited until graduation, the IEP team agreed on a goal centering on Jeremy applying for scholarships. April 15 is the deadline for most of the scholarships; however they will investigate to see if any additional funding is
