Goal 1: I will call at least three patients a day in order to acquire information about their medication adherence and to offer tools and advice on how to best manage their medications.
Plan: I will set aside time each day to complete these calls. I will suggest interventions such as weekly pill boxes, multi-dose packaging, or changing the timing of a dose. I will use the RIM model for patient adherence to determine the best ways to help my patients.
Assessment of progress: I called almost every day of my IPPE. I called patients about their “Meds on Time” a medication synchronization program that my preceptor developed. I called to see how their current medications were working and to see if they were ready for refills. I educated many individuals on medication adherence. I believe I averaged four calls a day.
Goal 2: I will analyze the needs of the patients at the pharmacy and develop an educational brochure suited specifically for them within my first two weeks.

This is why you have a preceptor. I felt very unsure of my knowledge. It was really difficult to remember every little thing that I had studied for the drug card tests. I wish that I had learned some of the therapeutics behind common medication classes. However, I was thankful that I knew how to counsel and how to communicate with different types of people. I felt prepared for these areas of the IPPE. Matt even mentioned how I counseled with more confidence than the last staff pharmacist that they had hired. Of course, I was also prepared for the data entry and dispensing process of filling
