Teleconsultation: Use of 24/7 teleconsultation is a safe, highly valued and effective means of delivering clinical support to people with COPD and care home residents, which leads to a substantial reduction in the need for both hospital admissions and Emergency Department attendances” (Pope, R., 2013). Currently, we have to ask the patients what medication they are on. Unfortunately some of the patient do not know their medication or the dosage. With continued innovation, this information will be accessible to the patient and the provider thus increasing compliance to treatment and care outcome. The challenge today is that there are different programs in use and the information is not shared beyond the institution and some of the users such as doctor’s office are still in the initial implementation stage (Meigs, S. l. & Solomon, M. 2016). There is need to agree on a shared minimum patient data that can be accessed in any care facility that the patient visits thus reduce medication errors such as administration of medication that patient is allergic to and was not updated on the accessed data. I see this happening in the next 5 years.
