Iowa Model Vs Star Model Analysis
Both the Iowa model and the Star model are the one of the process models, which describe the process of translation of theory or research-finding into practice (Nilsen, 2015). Both models are similar each other because they suggest the process of application of research-finding or evidence-based theory to clinical practice. However, while the Iowa model focuses on the organizational process, the Star model focuses on the knowledge transformation (Gawlinski & Rutledge, 2008).
Between them, the Iowa model was chosen to explain the current case with a couple of reasons. First, the first steps of both model are different. The process of the Iowa model starts with identifying the trigger factor, while the first step in the Star model is knowledge

An increasing problem in the organization (in the post-op orthopedic units) triggers the further investigation to solve this problem. Second, it seems that the Iowa model is more practical than the Star model because the Iowa model emphasizes the importance of problems in the organization, while the Star model emphasizes the transformation of knowledge (i.e. theory) to the practice. In the case, the current problem is the priority issue in the post-operative orthopedic adult and pediatric unit. However, the quality assurance and safety manager of the 286-bed facility is upset by this situation. Also, in both cases, there are several issues that nursing care was not systematic such as 1) the adult patient was not paid attention for 4 hours due to discharge schedule in the next day - 2:05am was the last time the nurse reported on …show more content…
As discussed above, the Iowa model suggests the processing steps to solve the problem in the organization. The current case was happened in post-operative orthopedic adult and pediatric unit, however, under the lead of Quality Assurance and Safety Manager, the hospital can assemble the team to conduct the EBP research to solve the current problem. Then, experts from different disciplines can make the significant contribution to the study. The Iowa model is also flexible, which the nurse can simply initiate the problem (trigger) and improve the practice through the internal research. Limitation of the Iowa model is that it suggests the interdisciplinary works to solve the problem, however, teams and groups are not always available. Another limitation is that the Iowa model starts with abstract ideas. Research team should make abstract ideas concrete to work with the problem.     Nursing sensitive indicators reflect the structure, process and outcomes of nursing care. The structure of nursing care is indicated by the supply of nursing staff, the skill level of the nursing staff, and the education/certification of nursing staff. Process indicators measure aspects of nursing care such as assessment, intervention, and RN job satisfaction. Patient outcomes that are determined to be nursing sensitive are those that improve if there is a greater quantity or quality of nursing care. ("Nursing world," 2013) Having knowledge of these indicators hard-wired and put into practice in daily care of patients, could have assisted the nurses involved in this particular case in many ways. Knowing the risk of pressure ulcer development with the use of restraints could have helped the nurse prevent one in this patient. The bedside care providers, the nurse, as well as, the CNA would have recognized the reddened area as the first stage in pressure ulcer development. They would have been more meticulous with skin assessments and repositioning with the use of restraints in this elderly patient. Also, if the CNA had a better understanding of when to appropriately use restraints and how to manage a restrained patient she wouldn’t have positioned the patient on his back after he returned to bed. Another nursing sensitive indicator is patient and family satisfaction which was not taken into account in this situation. The nurse so easily dismissed the daughter’s concerns about the dietary mix up and blew off her concerns regarding what her father was served.
