The client is currently seeing Dr. Delay for her medical needs. The client’s previous psychiatrist was Dr. Sayed; however, the client does not currently have a psychiatrist.

headaches last for 3 days at a time, and boredom due to her recent loss of her triplet sons in a custody battle. The client reports having thoughts of death; however, she denies intent, plan or a means to commit self-harm.
The client received outpatient services in 2013 with River City Comprehensive Services for symptoms of depression. The client was unable to recall the physician’s name.
The client admits to experiencing emotional and physical abuse by a previous boyfriend from 2005-2016.
The client lives in subsidized housing with her 8 year old son. The client does not have a history of homelessness or an eviction. During the assessment, the client was poorly groomed as evidence of her unkempt hair.
In addition, the client was not dressed appropriately for the weather. The client wore tank top and a pair of pajama pants.
The client appears to struggle with ADLs such as cooking, cleaning, shopping, budgeting her finances.
The client receives $300 in SNAP benefits and $200 in TANAF. The client does not receive any additional income to support incidentals that arise throughout the month.
The client has 4 son; ages 8 and 11. The client’s 8 year old son lives with her. The client’s8 year old son’s father has moved California and does not provide monetary support for the child. However, the client’s 11 year old triplets currently live with their father. During a temporary court hearing, held September 2016, it was decided by the court that the client’s 11 year old triplets live with their father. The client returns to court in May 2017 for a final hearing.
The client has a limited support system that consists of a close friend. The client’s family lives nearby.
Client’s current coping skills consist of holding her feelings and emotion to herself, listening to music and staying in bed.
The client denies a history of incarcerations or
convictions.
The client also denies the use of alcohol and/or substances; however, the client smokes approximately 1 pack of cigarettes daily.
