SUBSTANCE ABUSE: Client continues to deny any past or present history.
MENTAL HEALTH: Client denies mental health but she reported the previous worker scheduled her appointment to meet with the onsite psychiatrist doctor but she was no show.
Client Rights and Client Code of Conduct Violations & Facility Rules Violations: Client was no compliant for face to face meetings. CM reiterates the Shelter Rules and Regulation and for the client to adhere to the 10pm curfew.
WORKER ASSESSMENT:. Client has been non-compliant with meeting for face to face meeting since the case was assigned to this worker. Based on this worker observation client appears to have the ability to identify and complete goal in a timely fashion. Client has the ability to live independently.
PLAN OF ACTION:
Client must maintain all scheduled appointments, client must attend HRA Friends Field interview scheduled for 6/8/2016, and if hired submit work scheduled, client must adhere to the shelter rule and regulation including the 10pm curfew, client must maintain dorm and self-clean at all time.
